2008 Big Wheels Fundraiser Order Form

Box lunch meals, prepared by Jack's Gourmet Restaurant, will be sold for $10 each. The box lunches consist of Hoagie sandwich with ham, turkey, and two cheeses, pasta salad, a fresh fruit salad, soda and a cookie. A vegetarian wrap is also an option. We need three orders to deliver to an address but single orders may be picked up at Jack's, 1903 Business Loop 70 (across from Westlakes). Half of the cost is a tax deductible donation to Meals on Wheels and a receipt will be included in your lunch.
